Job Description
Job Description: Casual/ Part Time APP Weekend Hours

Provides limited care coordination and acute care (i.e., laceration repair, point of care testing, asthma management, immunization updates, simple splinting, etc.) to clinic patients with acute and chronic health conditions in collaboration with other medical providers or under the clinical supervision of a physician. Clinic patients include newborn to adult.  Responsible for the patient evaluation, interpretation, diagnosis, and prescribing of appropriate medical and therapeutic measures. Works autonomously within a clinic comprised of physicians, APP, CNM, medical assistants, RN, pharmacists, limited specialty services, and therapists. 85% or more of this job's practice is based within an urgent care setting. Seeking candidates on a part time basis for coverage of weekend and evening extended hours.   
 
Evaluates, diagnoses, treats, and manages acute patients within an urgent care setting for the majority of work shift.
Acts as the primary patient contact. Provides for continuity of care by ensuring smooth transitions between care settings and tracks patient along the care continuum including post-discharge plans. Provides clinical direction to nursing staff regarding the continued care of patients. Follows through with nursing staff to ensure patient receives recommended care. Prescribes, recommends, and regulates pharmacologic and non-pharmacologic therapies, including monitoring for previous controlled medications use and pain contract with limited prescribing of narcotic medications.
Provides care coordination, which includes managing a variety of ancillary services, internal or external referrals, communication with school for the school-based clinic patients. Orders, performs, and interprets procedures and diagnostic tests or studies, in the field of clinical specialty trained and within advanced practice provider scope and licensure, to manage simple conditions such as those produced by infection or trauma, as well as to assist in the management of more complex illness and injury. Examples could include injections, immunizations, suturing, wound care, and biopsies. Counsels, instructs, and educates patients on physical and mental health including diet, preventive health measures, health behaviors, self-care skills, disease, treatment options, and normal growth and development. Interprets and evaluates patient data within advanced practice provider scope and licensure for the purpose of determining management and treatment of patients. Consults with physician as appropriate. Adjusts patient care plan by reviewing and monitoring treatments and therapy plans. Prepares and signs patient care plans, encounter notes and summaries of patient care. Maintains records according to established standards and guidelines.

Scheduled Hours
2 or more shifts per month including 2 weekend days. Evening hours during the week may be available as well. (Searching for weekend and evening coverage)

Licenses & Certifications
Physician Assistant or Nurse Practitioner license issued by the state in which the team member practices, and
Family Nurse Practitioner certification issued by the American Academy of Nurse Practitioner Certification Board (AANPCB) or Physician Assistant (PA-C) issued by the National Commission on Certification of Physician Assistants (NCCPA), and Drug Enforcement Administration (DEA) registration issued by the Drug Enforcement Administration (DEA) Registration, and Basic Life Support (BLS) issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required.

Degrees
Master’s Degree in Physician Assistant Studies (may consider bachelor’s degree with 7+years relevant experience) or Master’s/Doctorate Nursing Degree in the specialty of Family Practice/Primary Care.

Required Functional Experience
Typically requires 2 years of experience in providing and coordinating care for patients with acute and chronic health conditions as a physician assistant or nurse practitioner.

Knowledge, Skills & Abilities
Maintains at least 85% of practice within an urgent care setting. Demonstrated ability to work independently and as an effective member of a health care team. Ability to work in a fast paced, high demands environment while managing multiple tasks. Excellent oral and written communication skills. Ability to effectively collaborate with and establish/build relationships with others. Proven organizational skills and ability to prioritize effectively.  Spanish speaking beneficial for patient population, translation services available.
