Job Description

POSITION SUMMARY:    Primary responsibility to seek out HIV infected clients who may be lost to follow-up and individuals who are not in care. In addition, provide targeted outreach to individuals and groups in the community related to HIV care and treatment services and other programs available at MHSI.

 

POSITION-SPECIFIC COMPETENCIES/ESSENTIAL FUNCTIONS/DUTIES & RESPONSIBILITIES

 
  • Locate clients lost to follow-up through referrals from MD, RNCM and SSCM. Assist client in re-entering care by bringing them into clinic. Assist client with making appropriate contacts i.e. Nurse or Social Service Case Manager.
  • Distribute HIV education/ prevention  materials i.e. pamphlets, brochures and condoms at community events health fairs etc. as appropriate
  • Educate community groups as assigned about Basic HIV information (i.e. transmission and prevention practices)
  • Submit daily detailed report of activities with objective and relative data.
  • Maintain accurate and complete records of outreach referrals.
  • Work collaboratively with EIP program staff by following up with appropriate EIP staff related to client referral.
  • Participate in continuing education activities as deemed appropriate.
  • Perform other duties as assigned.

 

POSITION REQUIREMENTS

 
  • Education:           High school graduate or equivalent.
  • Experience:          One year community outreach experience required.
  • Expertise:            Must be comfortable working with individuals from diverse populations and lifestyles. Basic working knowledge of HIV preferred. Knowledge of community based agencies and activities.  Local travel required. Must have reliable insured vehicle. Must be non-judgmental and be able to treat all patients with dignity and respect in a professional and courteous manner.
  • Language:            Comprehend and use basic language either written or spoken to communicate information and ideals.
  • Hours of Work:     May vary based on Organizational need.
  • Travel:                 May vary based on Organizational need.
