Registered Nurse Care Manager/RN, Part-Time, Day Shift,
Milwaukee, WI
Community Care is hiring a Registered Nurse Care Manager/RN for our Family Care Partnership Program. This is a part-time opportunity at our 3220 W Vliet St- Milwaukee, WI 53208 facility.
Registered Nurse Job Responsibilities:
Manage caseload of Partnership Program members
Complete comprehensive clinical assessments in community-based settings
Work collaboratively to develop, member centered care plans and provide on-going care coordination, member education, quality assurance and monitoring of services.
Regular local travel, requiring use of personal vehicle, valid driver's license, and
proof of required insurance coverage.
Registered Nurse Care Manager/RN Job Requirements:
Education:Associate degree in nursing required. Bachelor's degree in nursing preferred.
Licensure: Wisconsin Registered Nurse license required.
Experience:
Two years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity required
Two years of work in community nursing preferred
Registered Nurse/RN Job Schedule:
Monday to Friday (30 hours per week)
8:00 AM - 4:30 PM
