Job Description

 

Position Summary:

The Health Information Clerk assumes clerical responsibility and provides administrative support to the nursing staff.

Essential Duties and Responsibilities:

This list of duties and responsibilities is not all-inclusive and may be expanded to include other duties and responsibilities, as management may deem necessary from time to time.

·Helping maintain medical records for each resident that are complete

·Readily accessible and systematically organized

·Interacting with physicians, residents, staff, consultants, visitors and others in a manner that facilitates communication and presents appropriate image of the facility

·Assembles and analyzes discharged medical records in a timely and accurate manner.

·Answers the telephone promptly, courteously according to facility protocol. Takes telephone messages accurately and promptly notifies recipient of message.

·Makes prompt efforts to answer question, to resolve problems within scope of Unit Clerk responsibility or notify appropriate facility representative. Promptly notifies HIM Director or Nurse Manager of all and/or family concerns, grievances, complaints.

·Assists as needed in scheduling appointments and preparing documents for emergency resident transfer needs. Schedules Residents appointments within and outside the facility as directed by the Unit Charge Nurse.

·Assumes clerical responsibility for updating resident’s clinical records, such as plans of care and conference notes, according to facility policy when necessary.

·Maintains and updates current resource materials on units, including policy and procedures manuals, OSHA worker safety materials, exposure control plan for blood borne pathogens and other relevant materials as directed by Unit Manager.

·Maintains and promotes all professional aspects of HIPAA.

·Communicates with Nurse Manager throughout shift to coordinate efforts and to meet resident and staff needs.

·Participates in fire and disaster drills. In the event of an emergency carries out assigned duties to assure resident safety.

Uses personal protective equipment when indicated by exposure control plan; complies with all infection control procedures.

·Assists Unit Manager to prepare for State and Federal survey. Attends survey training and interacts with State surveyors as instructed by supervisor.

·Attends in-service education programs as scheduled. Applies information to job tasks.

Position Requirements:

·High school diploma or equivalent.

Working knowledge of ICD-9/ICD-10 and working understanding of legalities of the HIPAA regulations
Work Experience And Required:

·Previous clerical experience in institutional health care setting required.
Previous supervisor experience within an institutional health care setting require with prior supervisory experience in an HIM Department preferred.
Licenses & Certifications:

Preferred certificate in medical related field.

Required Skills, Knowledge and Abilities:

· Working knowledge of the facility record keeping policies and procedures and ability apply knowledge to job tasks.

·Sufficient interpersonal skills and maturity to communicate effectively with professional staff, direct care staff, physicians, consultants, residents, visitors and others in person and on the telephone.

·Ability to establish approximate priorities for receiving, recording, relaying and acting on information and to maintain organization on all unit clinical and administrative records.

·Ability to carry out all duties in accord with the facility mission and philosophy.
