The Clinical Pharmacist serves as a medication expert to support SSCHC providers and provide care to SSCHC patients. Engages with SSCHC clinical leadership and participates in decision-making processes related to the implementation of new clinical processes related to medication therapy. Engages with SSCHC clinical leadership to ensure appropriate use of clinic administered medications that meets Joint Commission standards. Oversee SSCHC Clinical Pharmacy Residents in conjunction with Schools of Pharmacy Assists in updating SSCHC 340B Formulary, educate clinic staff of formulary medications, and assists in review of formulary exemptions. Conducts Annual Wellness Visits for eligible patients. Updates SSCHC clinic staff on medication related issues, such as drug shortages, recalls, and guideline updates.
Required Qualifications:
Licensed as a pharmacist in the State of Wisconsin, with license in good standing
Minimum 3 years’ experience as a clinical pharmacist
Previous experience acting as site coordinator for a clinical pharmacy residency program
Participation in a Pharmacy Residency Program
Insurable for liability coverage as a pharmacist
Proven high level written and oral communication skills
Self-motivated and independent, able to work without direct supervision
Ability to process large amounts of data
Ability to work well within a team
Ability to communicate clinical information in Spanish (preferred)
