Preceptor Home Health & Hospice is growing!
We have experienced a huge increase in nursing visits within the past several months, and we need your help!

MINIMAL ON CALL IS REQUIRED!
We currently have an on call nursing team and a weekend baylor nursing team
which means that aside from an emergent situation and an occasional holiday,
your nights and weekends are yours to enjoy!
We are a home health & hospice agency providing services in clients' homes from Green Bay to Burlington and from Waukesha to Lake Michigan. Our home office is located in Germantown and our branch office in Kiel.
We are currently in need of a Home Health RN Case Manager for Milwaukee and greater Milwaukee area.
Our clients reside in assisted living facilities, their private home, or wherever home may be. We bring the care to them.
Job responsibilities include, but are not limited to, the following:
- Admitting new clients to our services
- resuming client care after a recent hospitalization
- Performing routine nursing visits
- Providing wound care -- we take great pride in being a preferred provider for several local wound care clinics
- Setting up and reconciling medications
- Providing catheter care
- Providing IV care
- Excellent communication skills with clients, families, doctors, facility staff, and co workers is a must to be successful in this role
- Ability to work independently
- Good organization and time management skills are also critical
As a home health nurse, you will be spending time in your car, and you will have a level of autonomy in scheduling your visits. Charting is done electronically. Our goal is for the majority of all charting to be done at bedside to allow you a work/life balance. HomeCare HomeBase is our electronic medical records system.
Experienced home health nurses are preferred; however, applicants new to the home health field are welcome to apply. All staff receives thorough training to set them up for success.
