Community Care is hiring a Care Manager for to join our Family Care Program team. This is a full-time opportunity at our 795 East Geneva Street, Elkhorn - Walworth, WI facility.
Care Manager Job Responsibilities:
Manage caseload of members
Responsible for assessments and working collaboratively with a Registered Nurse/RN Care Manager colleague to develop member centered care plans, provide on-going care coordination, member education, quality assurance and monitoring of services. Travel, Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
Care Manager Job Requirements:
Education: Bachelor's degree in Social Work, Psychology or Human Services
Licensure: Eligibility for long-term care functional screener certification is required
Experience: 18 months of experience in a social service capacity or a Master's degree in Social Work, Psychology, or Human Services and one year experience with older adults or persons with physical and or intellectual disabilities in a social service capacity.
Care Manager Job Schedule:
Monday to Friday
8:00AM - 4:30PM
