Community Care is hiring a Registered Nurse - Home Care for our PACE program. Home Care RNs act as an integral part of the IDT (Inter-Disciplinary Team) and actively participate in development of Member Centered Plans. This is a Full-Time opportunity based at our Vliet site - 3220 W. Vliet Street - Milwaukee.

Registered Nurse - Home Care Responsibilities:

• * Monitor, delivers and documents skilled nursing care (Ex. wound care, med administration, post hospital assessments) in member's homes.

  * Oversees care and interventions delegated to LPN and PCW staff.

  * Completes Home Care Coordinator visits and participates in team and care plan meetings as part of the IDT (Inter-Disciplinary Team)

  * Assists members in maintaining optimum health; provides health education and counseling to members and caregivers facing chronic conditionsand end of life issues.

Registered Nurse - Home Care Job Requirements:

• Education: Associates in Nursing required. Bachelors in Nursing Preferred.

• Experience: A minimum 1 year of personal, clinical/internship and/or employment experience providing care or services for a frail or elderly population. Excellent clinical and assessment skills. Working knowledge of principles of specialized treatment and training techniques related to adults with physical and/or developmental disabilities and dual diagnoses. Knowledge of multiple systems of care for elder adults and persons
  with physical disabilities. Experience with Electronic Health Record. Preferred: Experience in a community-based or long term care setting involving the elderly, 2 years of community nursing.

Registered Nurse - Home Care Job Schedule:

• Monday to Friday

• 8:00 am - 4:30 pm
