Community Care is hiring a CNA to provide in-home services for our Program of All-Inclusive Care for the Elderly (PACE) members. This is a part-time opportunity at our 2457 S. Browns Lake Dr. - Burlington, WI

CNA - In-Home Responsibilities:

  • Assists in identifying and reporting signs and symptoms indicating physical, emotional or behavioral changes in members. Maintains accurate and current written records indicating ongoing documentation of services provided. Monitors the completion of in home visit records and collaborates with supervisor to address any issues that arise. Audits IHAT and Care Plan documents for consistency in conjunction with Home care Supervisor. Accurately enters PCW schedules in uConnect and makes changes accordingly.

  • Assures punctuality for members' scheduled visits and van rides. Escorts members to appointments as necessary.

  • Provides personal care and/or homemaking services outlined in the plan of care. Assists with laundry as needed.

Job Requirements:

  • Education: High School Diploma Preferred

  • Experience: A minimum total of 1 year of personal or employment experience providing care or services for a frail or elderly population. Preferred: Experience working with individuals with dementia.

  • Certification: Wisconsin Certified Nursing Assistant in good standing on registry

Job Schedule:

  • Monday to Friday, weekend and holiday rotation

  • 6:00 AM - 11:00 AM
