How You'll Make a Difference

Provide medical leadership and clinical expertise, and administrative competencies for all hospice and palliative care services throughout the Aurora system. Responsible for the management of clinical and medical care for patients in the hospice programs through participation in the Interdisciplinary Group (IDG) and coordination with the primary care physicians, attending physicians and other medical directors. Provides clinical management and oversight to patients in the hospice facilities.


Responsible for utilizing a high level of interpersonal, negotiation and communication skills to work collaboratively with hospice and home care personnel and management staff within Aurora at Home (AatH) and within Aurora Health Care as well as with physicians, patients/family members and the community and its leaders.

Assists AatH and Aurora Health Care in clinical program development including on-going education and staff development on related topics such as pain management, symptom control and palliative medicine.

Consults with attending physicians regarding pain and symptom control, providing appropriate and timely feedback with resolving conflicts with physicians regarding the provision of service outside hospice protocols, guidelines and procedures.

Reviews and confirms patient eligibility for hospice services assisting with determination of appropriate Level of Care and placement in a facility if needed.

Participates with the IDG members in the development of the individual plan of care within 48 hours of patient admission to hospice.

Follow-ups and consults with hospice team members on an on-call basis for medical management of hospice and palliative care patients, making home visits when appropriate.

Coordinates efforts with attending physician to provide medical care in the event the attending MD is unable to retain responsibility for the care.

Acts as a medical resource for the IDG, physicians and AatH personnel on hospice and palliative medicine related issues and concerns. Acts as a liaison to physicians in the community regarding hospice care and palliative care services.

Leads the development of palliative care services across the Aurora system.

Serves as a consultant to administration concerning difficult patient care and physician practice issues.

Qualifications
License & Certifications
Medicine and Surgery, MD-DO license issued by the state in which the team member practices.
Education
Doctorate Degree in Medicine.
Experience
Typically requires 7 years of experience in the medical practice of hospice and palliative care.

Demonstrates a refined level of interpersonal, negotiation and communication skills necessary to work collaboratively with hospice personnel and management staff and Aurora Health Care as well as with physicians, patients/family members and the community and its leaders.Demonstrates professional competence, dependability and medical and personal ethics.
