Job Description
  • Participate in the evaluation of client needs and strengths, assist in designing supports and interventions, interview and complete bio-psychosocial assessments on each client.
  • Provide counseling/crisis intervention; to individuals, couples, and families in individual and group modalities.
  • Provide linkage to other services or support systems in the community; work with a multi-disciplinary team.
  • Actively communicate client needs and problem-solving solutions.
  • Plan, recommend, and implement referrals of clients and their families to appropriate agencies for treatment and follow up.
  • Coordinates various and specific referrals to decrease duplication of services and increase cooperation between service providers.
  • Act as liaison with staff from contract counties, referral agencies, or other treatment programs regarding client referral, treatment, progress, and discharge recommendations.
  • Provide primary case management for assigned clients, including assessment, treatment planning, monitoring, and evaluations, which can include monitoring commitments and hold opens.
  • Develop and implement service plans that provide for appropriate mental health services to clients and their families.
  • Coordinate referrals to residential treatment programs including thorough assessment for all clients.
  • Fulfill all mandated reporting duties to the appropriate agencies and departments required.
  • Assist in program development and participates in program implementation.
  • Attends staff meetings, in-services and outside agency training sessions.
  • Maintain the confidential nature of client and business information. 
  • Comply with all local, state, and federal laws governing clients and the rendering of services.
  • Complete and maintain necessary documentation according to applicable requirements, codes and policies.
 

POSITION REQUIREMENTS

 
  • Education:     Master’s degree in Social Work, Counseling, or related field plus one year of clinical                          experience in an alcohol or other drugs (AOD) or mental health setting preferred.  Bachelor’s degree in Social Work, Counseling ore related field.  LCSW, LSW, or LPC preferred
  • Experience:          A minimum of two years’ work experience an AOD or mental health setting.
  • Expertise:             Proficient with Microsoft Office software or compatible word processing programs. Ability to type a minimum of 60 wpm. Dictaphone, note taking skills and/or shorthand, speed writing, machine transcription skills. Basic knowledge of office copiers, fax machine, and postage machine. Excellent verbal and written communication skills. Punctual, reliable, can work independently, self-starter, trustworthy, honest
  • Language:            Comprehend and use basic language either written or spoken to communicate information and ideals.
  • Hours of Work:     May vary based on Organizational need.
  • Travel:                 May vary based on Organizational need.  Valid driver’s license, use of a reliable vehicle to transport clients and families and documentation of required vehicle insurance.
  • Other:                  Ability to secure Act 33 and 34 clearances.
