Job Description
Milwaukee Health Services, Inc. (MHSI) is a Federally Qualified Community Health Center (FQHC) that operates from two (2) sites: The Isaac Coggs Heritage Health Connection at 8200 W. Silver Spring Drive and the MLK Heritage Health Center at 2555 N. Martin Luther King Drive. MHSI offers a range of primary health care services including Medical, Dental, Behavioral, Women’s Health, Benefit Determination, and Pharmacy. FQHCs service everyone regardless of income, or third party coverage, and seek to provide high quality care in accessible locations and at convenient times. Our particular sites primarily target the residents of Milwaukee’s north side, but service patients countywide. FQHCs are unique in their delivery of care, patient population and provision of services. FQHCs seek to provide quality care in an underserved area to the uninsured or underinsured populations that would otherwise not have access to care.

POSITION SUMMARY:    To provide comprehensive Registered Nurse Case Management and nursing care to maintain/improve the quality of life for individuals infected with HIV/AIDS. Coordinate comprehensive healthcare. Collaborate with other healthcare providers and community agencies. Independently develop, implement, coordinate and evaluate healthcare education in an ambulatory setting.

 

 

POSITION-SPECIFIC COMPETENCIES/ESSENTIAL FUNCTIONS/DUTIES & RESPONSIBILITIES

 
  • Responsible for providing intensive clinical nurse case management for clients living with HIV.
  • Work independently with clients to assess needs related medical, social and behavioral health services.
  • Complete initial and ongoing nursing assessments that support medication adherence, health literacy, and client engagement in medical care, while maintaining client confidentiality.
  • Collaborate with a diverse group of multidisciplinary care team members (physicians, nurses, social workers, community providers, etc.), to proactively plan and implement care and service plan goals. Monitor changes and respond to follow-up needs of clients, follow through on identified referrals in a timely manner, and communicate
  • Facilitate client access to healthcare and community resources. Assists client to develop natural resources and makes appropriate referrals related to barriers to healthcare, (i.e. housing, transportation)
  • Provide nursing care within the scope of practice and performs clinical duties and assist physician during clinic sessions. Maintain clinic cleanliness and order (i.e. stocking rooms, rotating medication, etc.)
  • Document all encounters and contacts with clients. Maintains and updates patient data in electronic medical records.
  • Collect and organize data and reports as appropriate for all grant reporting.
  • Participate in departmental and organizational meetings and programs, including but not limited to community outreach and health fairs.
  • Seek opportunities to increase clinical, professional and personal growth independently and through in-house education, conferences and community resources.
  • Provide in-house and community presentations related to HIV/AIDS education
  • Work collaboratively, constructively and cooperatively with team members.
  • Perform other job duties as may be assigned.
