Interra Health is currently recruiting for a Health Coach to work onsite at one of our clients in Westfield, WI.
As a health coach, the successful candidate will identify and evaluate participants health behaviors. The health coach will provide wellness coaching services to improve clinical outcomes and control health care costs through education and program implementation.
Responsibilities:
Coach participants in person.
Work independently and establish, monitor and achieve goals with minimal supervision
Responsible for recording and maintaining electronic medical records for individual participants
Plan, develop, communicate, implement, monitor, and evaluate programs and projects to promote health coaching and health improvement at the worksite.
Develop coaching education materials (presentations, handouts, etc.)
Develops strong relationships based upon trust with individual participants
Collaborates with physicians, chiropractors, nurse practitioners and other health care providers to assess, identify outcomes, plan, and sequence and evaluate participant services across the care continuum. Focus is on prevention, health promotion, maintenance, restoration and education.
Assists clients in identifying, prioritizing, and implementing choices.
Empowers others to reach their goals, through direction, encouragement, accountability and counseling
Provides advice and instruction for engineering fitness, good nutrition, and management of weight, stress, and health into a participant’s lifestyle
Conduct onsite health screens.
Commits to continued learning in all areas relevant to the position
Maintain and develop knowledge about various health topics and behavior change strategies
Execute other duties/projects assigned by Project Managers
Health coaching is done onsite, face to face. The focus of health coaching is on prevention, health promotion, maintenance, restoration and education.
The hours will vary. Standard hours will be 5:30 a.m. - 2 p.m., two days per week and 6:30 a.m. - 3 p.m., two days per week. During peak times - hours may vary to accommodate the third shift hours. with some nights.
The successful candidate will have a background in Health Promotion.
The following candidates will be considered: LPN's, RN's, candidates with an Associates or Bachelors Degree in a Health Promotion, Kinesiology , Health Studies, Nutrition Science, Exercise Science and/or other health related field.
Other qualifications/requirements include:
• Excellent organizational skills, including the ability to effectively and completely handle multiple priorities simultaneously in a fast-paced environment, the flexibility and the ability to quickly adapt to changes.
• 2 years of related health education/coaching experience.
• Data management and analytical skills, including the ability to use a computerized database and spreadsheets, to analyze and present data.
• Proficiency in Microsoft Word, Excel, and Power Point.
• Ability to work both independently as well as part of a team
• Ability to sit for extended periods of time on the telephone
• Ability to lift up to 50 pounds
• CPR Certified (or willing to certify within 90 days)
• CHES Certification a plus
