Position Description
1. Accurately perform and record vital signs, height and weight.
2. Accurately calculate and record weight gain/loss and fluid intake/output.
3. Maintain a clean and safe resident environment.
4. Recognize abnormal changes in body functioning and the importance of reporting
such changes to a supervisor.
5. Perform palliative and post-mortem care.
6. Perform routine care rounds on each assigned resident, at minimum, every two
hours (or more frequently as indicated).
7. Supervise, assist or provide ADL’s such as:
· Bath, shower, whirlpool, and bed-bath
· Shampoo, grooming, shaving, nail care and mouth care
· Dressing/undressing, application of splints/prosthetic devices
· Toileting, bowel/bladder training, incontinent care an peri-care
· Meals/snacks, hydration, proper use of feeding devices/techniques
· AM and PM care per facility policy
8. Accurately calculate and record percentage of meal intake.
9. Provide preventative skin care techniques, observe the resident’s skin condition and
report any changes to a licensed nurse.
10. Utilize proper techniques and devices to transfer and position/re-position a resident.
11. Demonstrate ability to manage and respond appropriately to resident behaviors.
12. Meet the residents’ mental health and social needs.
13. Is aware of developmental tasks and physiological changes associated with the
aging process.
14. Maintain/support the resident’s right to maintain personal choices.
15. Able to work effectively with the residents’ families and use them as a source of
emotional support.
