JOB SUMMARY Pulls patient medical records for scheduled appointments on a daily basis. Re-files records daily following completion of scheduled appointments. Assists clinical staff in locating missing patient records as necessary. Maintains the confidentiality of the contents of patient records and ensures patient records are dept in the Medical Records Department when not in use by clinical staff. DUTIES AND RESPONSIBILITIES 1. Pull charts for scheduled patients and walk-ins. 2. Control medical record locating by using chart request forms and out guides. 3. Pull records as needed for attending physicians, nurse triage and ancillary clinical staff as requested for processing patient treatment, payment and clinic operation needs. 4. Weekly checks assigned file area for misfiled records and, as necessary searches clinical areas for missing records. 5. Prints schedule for each day’s scheduled patient appointments and pull records accordingly. 6. Print out guide cards for each day’s scheduled patient appointments and uses out guide cards when removing patients’ records from storatge shelf to contraol medical record location. 7. Assists patients and clinical staff in processing release of information requests taken in person and/or over the phone. Forwards requests to Release of Information Specialist for completion. 8. Answer incoming phone calls related to patient’s medical records; taking complete and accurate messages for department staff and transferring other calls to appropriate areas as required. 9. Processes requests for patient immunization records. 10. Processes incoming departmental mail on a daily basis. 11. File all patient information including lab results, test results, correspondence, etc. Maintains patient confidentiality related to information filed in the patient’s record. 12. Faxes physician reports and orders to outside health care entities with supervisory approval and records disclosures on the Disclosure Log located in the patient’s record. 13. As necessary, thins patient charts making a second or third volume as needed. 14. Repairs old or damaged records on a regular basis and makes new records for new patients on a regular basis. Tracks medical record numbers assigned to prevent duplication of numbers. 15. Assists in purging older records and moving to storage area as necessary and shifts records as space is made available. 16. Picks up completed records for clinical areas twice daily and re-files completed and/or returned records on a regular basis throughout the day. 17. Perform other related job duties, as may be assigned. EDUCATION, TRAINING AND EXPERIENCE High school graduate or GED with additional classes in medical terminology, filing systems, etc. and/or at least six months experience in an ambulatory care or acute cared medical records department.. SKILLS Ability to follow basic numerical filing systems. Entry-level knowledge of computer keyboard and basic computer operation. Basic knowledge of office copy and fax machines, and other office equipment as required.
