The Supervisor of Home Care will be responsible for supervising clinical field staff, support staff and assisting with patient care activities for assigned area, as determined by the Manager of Clinical Operations. Acts as clinical resource to team members. May also assume responsibility for direct patient care as needed. Promotes quality outcomes, patient experience, and team building. The Supervisor of Home Care will support operations for our South Suburban communities: Calumet City, Lynwood, Dolton, Glenwood, Lansing, Posen, South Holland, Thornton, Calumet Park/Riverdale, and parts of Chicago.

Job Description

Major Responsibilities:

  • Promotes an environment conducive to the delivery of quality patient care. Assist with chart audits and assure action is taken. Identifies trends in audits and communicates results to Manager and team.

  • Assures field supervisory visits are accurately completed according to agency policy as necessary.

  • Provides direction to the staff with clear responsibility for the coordination of resources to efficiently support day to day operations.

  • Monitors and assures staff productivity according to established targets on a daily basis. Assures timely correction of charting deficiencies that impact revenue cycle and clinical operations by field team members.

  • Participates in the orientation of staff through on-going direct teaching, coaching, observation, review and precepting of staff. Assesses educational needs of clinical staff and assists in development and facilitation of in services for staff. Participates in Field Team Interview/Hiring and Selection. Hires, coaches and terminates direct reporting staff.

  • Coordinates and ensures 24 hour availability of field staff services and rotates on call coverage with Manager. In collaboration with the Manager, conducts ongoing process improvement. Conducts tracers/joint visits and provides on-going feedback to staff.

  • Ensures accurate, timely and thorough OASIS or Hospice Item set data cycle time.

  • Works collaboratively with operational staff and other leadership to ensure growth and maintenance of established patient and referral relationships.

  • Establishes and maintains quality standards, continually evaluating workflow and processes.

  • Develops collaborative and cooperative relationships with customers, physicians, community organizations and entities of Advocate Aurora Health at Home.

  • Performs human resources responsibilities for staff which includes coaching on performance, completes performance reviews and overall staff morale. Recommends hiring, compensation changes, promotions, corrective action decisions, and terminations.

  • Responsible for understanding and adhering to the organization's Code of Ethical Conduct and for ensuring that personal actions, and the actions of employees supervised, comply with the policies, regulations and laws applicable to the organization's business.


Licensure, Registration, and/or Certification Required:

  • Registered Nurse license issued by the state in which the team member practices, or

  • Physical Therapist license issued by the state of Wisconsin, or

  • Occupational Therapist license issued by the state of Wisconsin, or

  • Speech-Language Pathology license issued by the state of Wisconsin, and

  • A valid drivers license issued by the Division of Motor Vehicles.


Education Required:

  • Bachelor's Degree in Nursing, or

  • Bachelor's Degree in Physical Therapy, or

  • Bachelor's Degree in Occupational Therapy, or

  • Bachelor's Degree in Speech Pathology.


Experience Required:

  • Typically requires 3 years of experience in clinical nursing or rehabilitation.


Knowledge, Skills & Abilities Required:

  • Must have thorough understanding of home health or hospice regulations, reimbursement and care requirements.

  • Strong communication skills.

  • Demonstrates an ability to organize, plan and delegate work effectively an efficiently.

  • Demonstrates an ability to resolve conflict, motivate staff and interact with superiors, peers and subordinates in a direct and professional manner.

  • Demonstrates an ability to solve problems and make independent judgments and decisions based on experience and good judgment.


Physical Requirements and Working Conditions:

  • Exposed to a normal office environment.

  • Position requires travel therefore may be exposed to road and weather hazards.

  • Operates all equipment necessary to perform the job.
