Department: Out- Patient Physical Rehab Services

Location: Rolling Meadows, IL

Hours: This position is 20 hours per week. Must be able to start at 8:00 on Wednesdays. Other part of schedule can be flexible! Can work 3 days or 4 days depending on preferred choice. No Weekends and No Holidays!

Shift: Days

Part time/Full Time: Part Time 20 hours per week

General Summary:

• Provides Counseling Services

• Reporting to the assigned Unit Manager, coordinates matrix relationships that involve collaboration with multiple healthcare team members.

• Provides consultation and support to the team members on patient resource and psychosocial matters and concurrent review issues upon request.

• Identifies community resources, coordinates financial and clinical aspects of health care

• Maintains accountability for process improvements that reflect increased quality and decreased cost of care.

• Will serve as patient advocate

• Functions as an integral member of a collaborative and interdisciplinary team

What You will need:

• Master's Degree from an accredited school of Social Work.
• Licensure to practice social work in Illinois (LCSW)
• Two years' post-graduate experience preferred; healthcare and/or mental health experience preferred
• CPR from American Heart Association
