Community Health Worker - NorthShore

Position Highlights:

  • Position: Community Health Worker (CHW)

  • Location: Evanston, IL

  • Full Time/Part Time: Full Time

  • Hours: Monday-Friday, day shift. In addition, there may be occasional evening or weekend hours for community events.

  • Travel: Frequent local travel will be required. You must either maintain a valid State of Illinois Driver's License with proof of active insurance or have access to other means of transportation.

What you will do:

  • Assess and address patient's needs in the area of housing, food insecurity, transportation or other non-medical areas that influence a patient's health outcomes (Social Determinants of Health).

  • In partnership with patient care teams, provides support for patients in gaining access to medical services, screenings, prescription medication and durable medical equipment.

  • Assist patients with getting access to social services and other community-based resources.

  • Follows up with patients via phone calls, home visits and visits to other settings where patients can meet.

  • Maintain accurate documentation of patient contacts and document patient's needs, action plan and outcomes in the medical record system.

  • Represents the organization at various community events.

  • Participate in continuing education, trainings and CHW organizations.

What you will need:

  • Education: High school diploma or GED

  • Experience: Minimum of 2 years of community based experience.

  • Skills:

    • Strong passion, leadership and interpersonal skills necessary to effectively work with multi-cultural, diverse populations.

    • Ability to effectively engage in conversations, motivate and encourage patients to take steps to maintain and improve their health.

    • Creative thinker with ability to problem solve and brainstorm solutions.

    • Ability to work collaboratively as a part of a patient care team.

    • Ability to work independently with minimal supervision.

    • Leadership and community education experience.

    • Basic computer skills required. Experience with EPIC EMR preferred.

    • Fluency in Russian, Spanish, Urdu, Hindi, or Arabic a plus.

  • Certification: Community Health Worker, Health Coach, Patient Advocate Certification and/or equivalent preferred.
