The Behavioral Health Therapy Resource Coordinator provides clerical, administrative and case management support to the Behavioral Health Therapists (BHT's) to facilitate discharge plans, collaborate with established outpatient teams, refer patients to additional treatment/care, connect patients and their families to additional resources, and complete other related activities, as assigned. This role coordinates resources, referrals, and appointments for patients/patient families and supports the Behavioral Health Therapy team with projects and clerical tasks.

Responsibilities:
  • Provides support for patient and/or family transportation arrangements and coordinates communication with transportation companies, as needed.
  • Monitors worklist for tasks as assigned by the BHT, psychiatric provider, or RN CM.
  • Identifies insurance providers or benefits for patients.
  • Coordinates, records, and transmits information pertinent to the resource management of patients and/or their families.
  • Reviews copies of legal documents as it relates to the role of care coordination and scans into the medical record as appropriate.
  • Contacts therapists, case managers, and members of the outpatient team as it relates to care coordination and discharge planning.
  • Invites all necessary parties to the patient staffing, as needed/requested by BHT's.
  • Coordinates with providers, BHT's, and Occupational Therapy team for recommendations on additional services to patient discharge plans (e.g., DD/ASD, CCS, residential treatment, IOP/PHP, etc.).
  • Documents outpatient follow-up in the electronic health record.
  • Obtain Release Of Information (ROI) for any new providers or as otherwise needed.
  • Coordinates all outpatient referrals, completes required forms and schedules follow-up appointments for patients (appointments scheduled with outpatient therapists within seven days of discharge; appointments scheduled for medication management within 2-3 weeks of discharge, as possible).
  • Makes discharge phone calls within 72 hours of discharge.
  • Creates contact sheet for Behavioral Health Therapy team that consolidates information from electronic and paper charts.
  • Collaborates/networks with outpatient clinics and resources to ease referral processes.
  • Maintains an up-to-date database of community resources and providers.
  • Maintains an up-to-date database of Smartphrases for use in electronic medical record.
  • Accurately tracks and orders department resources such as medication lock boxes, trigger locks, and patient library books.
  • Manages the Staffing and Family meeting database.
  • Schedules meetings and room reservations at the request of the BHT team.
  • Supports staffing communication within the department as appropriate.
  • Enters information in databases and reports as directed by the BHT team.
  • Maintains the Discharge Phone Call database.

Required Qualifications:
  • Bachelor's Degree in Social Work highly preferred, but may consider other degrees in lieu of BSW if candidate has case management experience.
  • One-year experience in mental health care or social service setting
  • Experience with Microsoft Office
  • Knowledge of medical terminology
  • Excellent verbal and written communication skills
  • Ability to multi-task and prioritize workload
  • Flexible and adaptable to changing environment
  • Excellent critical thinking and problem-solving skills
  • Positive attitude with team-oriented approach

Preferred Qualifications:
  • Master's Degree in Social Work
  • Experience working in case management
  • Experience with electronic health record
  • CPR
  • Experience with health care agencies and community resources
  • Knowledge of payer environment
