Job Description

To safely provide Occupational Therapy Services to the patient/resident/clients of the facility. To be an active member of the interdisciplinary team.

Key Responsibilities:

  • Provide individual and group therapy to patient/resident/clients as ordered by physician and directed by a Registered Occupational Therapist.
  • Provide assistance to Occupational Therapist on Occupational Therapy evaluations of patient/resident/clients.
  • Provide consultation to program staff on problems related to Occupational Therapy, specific treatment techniques, and other programming issues to promote understanding of Occupational Therapy principles and team involvement in patient/resident/client care.
  • Assume responsibility for supervision of Occupational Therapy Assistant Students as assigned.
  • Document all clinical services provided.
  • Conduct job duties in a manner to ensure safety of patient/resident/clients, families, and staff.
  • Adhere to Administrative, Clinical, and Human Resource Policies of Lakeview.
  • Provide education to patient/resident/client and family as indicated.
  • Demonstrates clinical competence in all aspects of patient care related to occupational therapy.
  • To be proficient in the EMR Welsky within 90 days of hire.


Qualifications:

  • Skill, Knowledge and Abilities: Knowledge of neurological evaluation and how to conduct Occupational Therapy. Must be able to communicate both in writing and orally.
  • Education: Occupational Therapy Assistant degree and AOTCB certification.
  • Licensure: Licensed by the State of Wisconsin Dept. of Regulation and Licensing to practice Occupational Therapy.
