Job Description

JOB PURPOSE AND REPORTING STRUCTURE: Under the direction of the Medical Director/Chief Medical Officer, the Nurse Practitioner provides primary care medical services to patients in accordance with agency policies and procedures.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Evaluate, diagnose and manage acute and chronic medical conditions, seeking consultation as necessary
  • Order, perform and interpret laboratory test including diagnostic and invasive procedures.
  • Perform diagnostic and therapeutic procedures within provider scope
  • Provide preventative care and lifestyle counseling
  • Provide care with consideration of the patient's social, financial and cultural context
  • Prescribe medicine as necessary and appropriate in accordance with states and federal laws
  • Provide after-hours clinical advice for OCHC patients when assigned.
  • Assume quality of care for all patients.
  • Adhere to the highest standards of medical practice, ethics, and professionalism at all times.
  • Collaborate with physician in establishing and reviewing drug and other medical guidelines. Review of guidelines will be done in a continuing manner.
  • Assess and discuss medical and nursing diagnoses including therapeutic or corrective measures employed in a continuing manner when the dual management method of care is employed.
  • Maintain professional and technical competency and remain current relative to changes in the industry
  • Become familiar with the Federal and State rules, regulations and practices in a Community Health Center
  • Complete special projects as assigned.
  • Ensure the completion of all required paperwork, records, documents, etc.
  • Ensure compliance with all safety and work rules and regulations. Ensure the maintenance of departmental housekeeping standards.

EDUCATION and/or EXPERIENCE: Requires a Masters in Nursing and a minimum of five to seven years of related experience with applied knowledge of medical and nursing terminology, and nursing practices and procedures; or a combination of education and experience.
