Reporting to the Supervisor Case Management, the Social Worker is responsible for coordinating the care of designated patients and families during a patient's hospital admission. The Social Worker coordinates the discharge plan from pre admission through discharge follow up. The Social Worker provides psychosocial assessment and updates that address the patient's evolving

Observes patient in inpatient setting to ensure their comfort and safety. Communicates closely with the registered nurse to have a better understanding of the patient's needs, diagnosis, visual, auditory or language barriers, along with any other special considerations. Makes home visits to specified patients and provides personal care according to the individual plan of care developed by the registered nurse. Performs routine personal care activities for patients, which includes but not limited to; bathing, dressing and undressing, skin, nail, hair, osmotic, and simple wound care, shaving, oral hygiene, and care of catheter and drainage
