Horizon Home Care & Hospice, a private, not-for-profit organization, was established in 1990 and today serves as the second largest full service home care agency and hospice in Wisconsin. We are both hospice and home care licensed, Medicare and Medicaid certified and accredited by the Joint Commission on Accreditation for Healthcare Organizations. We serve patients in southeastern Wisconsin from our two locations, Milwaukee and Hartford, averaging a census of over 1,000 patients served by a staff of over 525 employees.
