Outreach Community Health Centers, Inc. is a Federally Qualified Community Health Center.  The health center is patient directed and serves populations with limited access to health care.  Outreach Community Health Centers, Inc. has either directly provided or coordinated the provision of health and social services for the homeless or those at imminent risk of homelessness since its incorporation in 1980.  As Health Care for the Homeless of Milwaukee (HCHM), we were one of the original 23 homeless demonstration projects funded by the Robert Wood Johnson Foundation and the Pew Memorial Trust.


MISSION STATEMENT: To ensure that men, women and children in the Greater Milwaukee Area receive the quality health care, behavioral health, housing and supportive services they need to live at the highest possible level of self sufficiency.


Outreach Community Health Centers, Inc. will work to achieve this mission through advocacy, outreach, prevention, public awareness, collaborative relationships with other services providers and contracting for or providing a quality, cost-effective continuum of care.
